Narrative verdict recorded at inquest

A narrative verdict has been recorded at an inquest into the death of a patient at Prince Phillip Hospital, Llanelli.

David Anthony Jones died in his hospital room on September 20th 2010 within an hour of a suicide watch being relaxed.

Dr Carl Hooper, Clinical Programme Lead for Mental Health and Learning Disabilities, Hywel Dda Health Board said: “On behalf of Hywel Dda Health Board, I would like to extend my sincere sympathies and deep regret to the family of Mr Jones.

“We have taken very seriously the need to question every aspect of our service to understand how this happened and how we might prevent it from happening again.

“I would like to reassure the family and wider public that changes have already been put in place to ensure the safety of patients in our care.”

A narrative verdict is a verdict available to coroners in England and Wales following an inquest. In such a verdict the circumstances of a death are recorded without attributing the cause to a named individual.
